Handicapped Parking Permit Application

Instructions and Application for a Parking Permit or License Plates, for People With Severe Disabilities NYS DMV MV-664.1 /664.3 

You must deliver the application to the Police Department in person. In addition, you must have your driver's license or non-driver ID number from your NYS DMV issued license or ID with you when you are present at Headquarters.
